Financial Policies
All fees, including co-payments and co-insurance payments, are due at the time of service. Payment is accepted in the form of cash, major credit card, or CareCredit.
Office Policies
If you miss your appointment or cancel your appointment without advanced notice, it may prevent Dr. Dobecki from helping other patients in need. In consideration to other patients and to our practice, kindly give 24 hours notice if you are unable to keep your appointment. Noncompliance with this policy will result in a late-cancellation or missed appointment fee before scheduling another appointment.
Patient Forms
If you are a new patient OR if you have not been seen by Dr. Dobecki in over a year, please download and complete the forms on our website prior to your visit. You may also call the office to have these forms faxed or mailed to you. In order for Dr. Dobecki to maximize time spent with you on the day of your visit, it is recommend that you complete these forms in advance. If you are unable to do so, then please arrive 20 minutes prior to your appointment to complete these forms.
