If you are a new patient OR if you have not been seen by Dr. Dobecki in over a year, please download and complete the following forms prior to your visit. You may also call the office to have these forms faxed or mailed to you. In order for Dr. Dobecki to maximize time spent with you on the day of your visit, it is recommend that you complete these forms in advance. If you are unable to do so, then please arrive 20 minutes prior to your appointment to complete these forms
